We're here to help you with well-child visits, vaccines, illnesses, behavior problems, learning disorders, and to answer questions all parents have at one time or another. If your child has a chronic illness, we're here to provide you with a medical home and make sure he or she gets the attention and care they need.

Cook Children's Emergency Department is supported by one of the country's largest pediatric transport programs. More than 2,000 children are transported to Cook Children's Medical Center annually, most for critical care.
Our doctors are specialists in the health care of infants, children and adolescents. Our team includes board-certified pediatricians.
Dr. Jackson received her medical degree from Howard University College of Medicine, Washington, D.C., and her residency at University of Maryland Medical Center, Baltimore, MD. She is board-certified in pediatrics.

Dr. Leung received her medical degree from University of Texas Southwestern Medical School, and her residency at Texas Children's Hospital/Ben Taub General Hospital. She is board-certified in pediatrics.
Dr. Radimecky received his medical degree from University of Texas Medical Branch, San Antonio, and his residency at University of Oklahoma Children's Hospital. He is board-certified in pediatrics.
Dr. Wheeler received his medical degree from Vanderbilt University Medical Center in Nashville, Tennessee, where he also completed his residency. He is board-certified in pediatrics.

We know that filling out forms can be time consuming, however, they are a required part of your child's care. For your convenience, we've provided patient information and consent forms here. Just print them out, fill in the information, sign them and bring the completed forms with you when you come in for your appointment.
