We're here to help you with well-child visits, vaccines, illnesses, behavior problems, learning disorders, and to answer questions all parents have at one time or another. Cook Children's Emergency Department is supported by one of the country's largest pediatric transport programs. More than 2,000 children are transported to Cook Children's Medical Center annually, most for critical care.
Our doctors are specialists in the health care of infants, children and adolescents. Our team includes board-certified pediatricians.
Dr. Olvera received her medical degree at the University of Texas Medical Branch at Galveston. She completed her pediatric residency at Texas A & M University College of Medicine, as well as a fellowship at the University of Texas Southwestern Medical Center in Hematology/Oncology. Dr. Olvera attained her board certification from the American Board of Pediatrics in 2000 and became board-certified in Hematology/Oncology in 2004. She is married and has four children (two sets of twins).

Dr. Ozaeta received his medical degree at the University of Texas Medical School at Houston and completed a pediatric residency at the University of Texas Health Science Center, also in Houston. Dr. Ozaeta attained his board certification from the American Board of Pediatrics in 2001. Prior to joining Cook Children’s, he practiced with the Las Colinas Pediatric Association in Irving. He is married and has four children.
Dr. Rafati received her undergraduate degree from Harvard University in Cambridge, Mass., and earned her medical degree from Baylor College of Medicine in Houston. She also completed her pediatric residency and accepted a faculty appointment as a clinical instructor at the Baylor College of Medicine. Dr. Rafati served as the neonatology chief resident at Ben Taub General Hospital in Houston. Dr. Rafati is fluent in Spanish.

We know that filling out forms can be time consuming, however, they are a required part of your child's care. For your convenience, we've provided patient information and consent forms here. Just print them out, fill in the information, sign them and bring the completed forms with you when you come in for your appointment.
