Insurance information
Your doctor’s office will provide us with your insurance information. If you know this information has changed - please contact our Business Office at 972-300-0215 with the current information.
Our Business Office personnel will call your insurance to verify coverage, which will include copayments, coinsurance and deductibles. Our calculation of your out-of-pocket costs for your child’s procedure will be based on this information. You will receive a call prior to the surgery date to explain these estimated costs and it will be your responsibility to pay them on the date of service.
We accept MasterCard, VISA, American Express and Discover cards. If you have questions regarding these costs, please contact our Business Office at 972-300-0215.
