Preparing For Your Visit

Prepare for a new appointment

It is important that we have your child's referral or doctor's order when you come for a new appointment. You may fax it in advance or bring it with you. A patient packet will be mailed to you or you may also print out the appropriate forms below. We request that you fill this out in advance and bring it with you to your first appointment.

It is strongly recommended that you check with your insurance plan regarding your Rehab benefits. Cook Children's rehabilitation is billed as a hospital-based outpatient service.

You will need to bring:

  • Driver's license
  • Patient's insurance card

You should receive an automated confirmation/reminder call 48 hours before your scheduled appointment. The message may include such details as the patient's name, the date/time of the appointment and the location.

Your first visit

At the first visit, a licensed therapist will test your child as part of the initial evaluation.

Then, using the test results from the first therapy visit, they will develop a treatment plan. Family input is an important part of the treatment plan. Families help set goals for home programs. The therapist will advise what therapy your child needs.

After the test is done, your therapist will write a report. Results, goals and a therapy plan will be in the report. Our staff will send the report to your insurance company. We will ask for approval for therapy visits. Our staff will also mail a copy of the insurance letter to your home. After your insurance is approved, we'll call you to make the first appointment.

What happens during therapy appointments?

  • Therapy activities are based on your child's treatment plan. We want you to participate in these activities.
  • We'll track your child's progress in the medical chart.
  • We'll teach you how to do therapy with your child at home.
