Location, Transportation and Driving Directions

Have you ever felt a bit uncomfortable in a doctor's office? When we designed our offices, one of our main goals was to make it as warm and inviting as possible, from the colors and lighting all the way to the seating so that you and your children feel comfortable. Our staff is warm and loving, speaks both Spanish and English, and knows how to make a kiddo feel welcome!

Medicaid Transportation - LogistiCare

If your child is covered by Medicaid or CSHCN, you can request transportation help to and from your child’s clinic appointment from LogistiCare. LogistiCare takes request for routine transportation Monday through Friday from 8 a.m. to 5:00 p.m. Submit requests for routine transportation 48 hours (2 business days) in advance of the trip. Requests for Advanced Funds should be submitted at least 5 business days in advance of the trip.

Hours and appointments

  • Please call in advance for an appointment.
  • Call between 8:00 a.m. – 5:00 p.m., Monday – Friday. We are better able to answer your questions and give you advice when we can have your child's medical records on hand.
  • This location is not an urgent care clinic. Find Cook Children's urgent care clinic locations near you.
  • We see children for sick visits after hours, Monday through Friday, 6 p.m. – 10 p.m.
  • Routine office visits are by appointment only; we will make every effort to accommodate your needs when your child is ill.
  • Please call as soon as possible if you must cancel or reschedule your appointment.
  • Our offices are closed most holidays, including New Year's Day, July Fourth, Thanksgiving and Christmas.
