The idea for medical homes was initially introduced by the American Academy of Pediatrics in 1967. Today, it's recognized as a premier program for pediatric and adult primary care practices. The patient-centered medical home is a way of organizing care coordination and communication to transform primary care into what parents and our young patients want it to be. Medical homes can lead to higher quality and lower costs, while at the same time improving the partnership between patients, families, and the medical team.
