Appointments and Referrals

New and existing Cook Children's Clearfork pediatrician office patients can make an appointment by calling 817-336-4040.
Call to schedule appointment
  • New patients, please have your insurance information ready when you call.
  • Our office is closed on Sundays.
  • Call between 8 a.m. – 5 p.m., Monday – Friday. 8 a.m. – 12 p.m., Saturday and holidays. We are better able to answer your questions and give you advice when we can have your child's medical records on hand.
  • A nurse will take your call, or if one is unavailable, leave a message with a receptionist and a nurse will return your call. Rapid responses are very important to us.
  • Routine office visits are by appointment only; we will make every effort to accommodate your needs when your child is ill.
  • Please call as soon as possible if you must cancel or reschedule your appointment.
  • Our office is closed most holidays, including New Year's Day, July Fourth, Thanksgiving and Christmas.

  1. Login to your MyChart account and select the child you wish to schedule an appointment for.
  2. Select "visits" from the top toolbar, then select "schedule an appointment" from the dropdown menu.
  3. Select an option from "schedule with a provider you have seen before" or "tell us why you"re coming in". Follow the prompted questions and next steps.
  4. Complete the additional details section with a brief summary of the reason for the appointment.
  5. Select your desired date and appointment time and click 'schedule'.
  6. Your appointment has been scheduled! You can view your upcoming appointment under "visits- appointments and visits" and you will receive reminders for your upcoming appointment.

Using the Mychart App

  1. Login to your MyChart account via the app and select the child you wish to make an appointment for.
  2. Select "appointments" calendar icon, then select "schedule an appointment" green icon.
  3. Select "schedule with a provider you have seen before" or "tell us why you"re coming in". Follow the prompted questions and next steps.

  5. Select your desired date and appointment time and tap 'schedule".

MyChart video visits:

Cook Children’s pediatricians are partnering to provide online video visits to help provide the quality pediatric care you expect from Cook Children's. These visits can be done from the comfort of your own home on a smartphone, tablet or computer with a webcam. In preparation for a visit:

To schedule a video visit with your pediatrician, follow the below steps:

  1. Request a visit by selecting the "Appointments" tab in MyChart or by calling your pediatrician’s office to set up an appointment. These appointments will be available during regular office hours.
  2. When the time comes for a visit, you will go back to the “Appointments” tab in the MyChart app to begin your visit.

In addition to telemedicine visits, Cook Children’s pediatricians are actively working to help keep patients safe by direct messaging with patient families and providing phone visits when video is not an option. Messages can include a picture or video and can be sent through the MyChart “Messages” tab. Talk to your office and /or doctor to confirm if they are providing these options for their patients at this time.

eCheck-In is now available!

If your child has an upcoming appointment that requires forms and information to be completed or updated, you can now do so before your appointment, in your MyChart portal account.

Simply login to your child's MyChart account, select 'Visits' and 'Appointments and Visits' and select the green icon "eCheck-In". From there, you can fill out the necessary forms for your child's upcoming appointment, saving you time in the office.

For your convenience, we've provided patient information and consent forms here. Just print them out, fill in the information, sign them and bring the completed forms with you when you come in for your appointment.

Request medical records

Cook Children's keeps a record of each lab, x-ray, medical center, or specialty clinic visit. The privacy of this information is protected by federal and state regulations. This means your medical information is only available to the Cook Children's healthcare professionals caring for you or your child. Everyone else must get special permission (called authorization) before the information can be released.

Specialty care referrals

At Cook Children's, we provide more than 60 pediatric medical and specialty clinic offices to support your child if they require specialized care. We offer child-friendly medical and surgical technology, as well as care designed to help children and their families feel at ease and safe during a stressful time.

  • Referrals to a specialist may be made by your doctor. However, no referrals are made until your child has been seen by his or her primary care physician.
  • If you are a member of a managed care plan (HMO), a referral form is required for treatment by a specialist. Allow five to seven working days for the referral to be completed and mailed.

Home health services

Sometimes kids need long-term care, or will heal better in a familiar setting. We can assist in arranging referrals for quality, patient-centered care in the comfort of home through Cook Children's Home Health. Led by a clinical team with vast experience in pediatrics, our visiting nurses have a minimum of five years of pediatric experience and our skilled visiting nurses are PALS certified.

Fees, payments and insurance

  • Fees, insurance co-payment and deductibles are due at each visit.
  • Please bring insurance identification to every appointment.
  • Let us know when there are changes in insurance coverage, address, employment or phone numbers.
