Services
Cook Children's Palliative Care team follows patients with a wide variety of serious medical conditions, including, but not limited to:
- Genetic and metabolic disorders
- Prematurity
- Congenital malformations and syndromes
- Severe cerebral palsy
- Neurodegenerative or neuromuscular conditions
- Advanced cancer
- Severe heart and lung disease
Many of our patients and families are coping with the burden of uncertainty regarding the future. Our team supports them, as they hope for the best, attend to the present, celebrate success and prepare for the rest – including possible worst-case scenarios.
Trusted expertise: Our doctors are board-certified, which means they are trained and experienced in caring for your child. From the moment you contact us, you'll experience our deep commitment to your family.
Timely care: We offer virtual video visits for certain appointments and can answer your questions by phone.
Coordinated specialty services: If your child has a more complex medical need requiring additional specialists, we can refer you to nationally recognized teams in the Cook Children's network. We partner with more than 60 other specialty clinics to ensure every child gets the care they need.
Easy access: Our MyCookChildren's patient portal makes it easy to stay in touch. Message your doctors with a question. Schedule an appointment or see test results. You can even send a picture or video for immediate concerns.
Your visit
If you have been referred to Cook Children's Palliative Care program, there are some things you can do ahead of time to help make your visit more comfortable. The following information is provided to help reduce anxiety and unnecessary stress for both you and your child.
Before your visit
Prior to your visit, you will want to contact your managed care provider with questions about your insurance coverage, co-payments or deductibles. You may also be contacted by a patient registration representative with benefit information for your service. Depending on your situation, you may want to meet with someone from our social services department for assistance with financial issues and, if you're coming from out of town, information on transportation and accommodations.
What you'll need to bring
Bring the following information to your child's appointment:
- Your photo identification and insurance card(s).
- Paperwork for referrals, authorizations and physician's orders.
- You will be asked to pay any deductibles, co-payments or unmet out-of-pocket expenses that apply. For your convenience, most major credit cards are accepted.
Upon arrival at the medical center
Please check-in at Patient Registration on the day of your appointment to complete the registration process and receive directions to your appointment location
