Preparing for your first visit
- Be sure to create a MyCookChildren's patient portal account to access your child's health records online.
- Bring your insurance card and other medical history.
Fees, payments and insurance
- Fees, insurance co-payment and deductibles are due at each visit.
- Please bring insurance identification to every appointment.
- Let us know when there are changes in insurance coverage, address, employment or phone numbers.

Are you an existing patient and still need to sign up for your MyCookChildren's account? There are 3 ways to sign up:
- Call 682-303-4367.
- Visit MyCookChildren's to fill out a form requesting an account.
- Register in person at your child's next visit.
Access Cook Children's MyCookChildren's patient portal to exchange messages, review and pay bills, request appointments and insurance information.
Patient information and consent forms
Confirm and update patient information in your MyCookChildren's patient portal.
If your child has an upcoming appointment that requires forms and information to be completed or updated, you can now do so before your appointment, in your MyCookChildren's portal account.
Simply login to your child's MyCookChildren's account, select 'Visits' and then the green 'eCheck-In' icon for your child's upcoming appointment. From there, you can fill out the necessary forms needed. This process from start to finish should take less than 20 minutes and will shorten your time in our office. Please note: the forms and questionnaires are specific to who your appointment is with and what is needed for that type of appointment. Even if you recently updated forms, there may be new items to complete that are specific for this appointment.

ADHD forms and appointment prep
If your child is visiting for an ADD/ADHD evaluation, we encourage you to download these forms and have them ready for your visit.
Virtual Waiting Room
As an added layer of protection, we now offer Virtual Waiting Room. On the day of your appointment, you'll receive a text with instructions about your visit. When you arrive, text us that you're here and remain in your car until you receive a text from us letting you know it's time to come in.

Scheduling an appointment online
Need help scheduling an appointment online? We’re here to help! See below for step-by-step instructions on how to schedule an appointment with your provider online.
Desktop and mobile browsers
- Login to your MyCookChildren's account and select the child you wish to schedule an appointment for.
- Select "visits" from the top toolbar, then select "schedule an appointment" from the dropdown menu.
- Select an option from "schedule with a provider you have seen before" or "tell us why you"re coming in." Follow the prompted questions and next steps.
- Complete the additional details section with a brief summary of the reason for the appointment.
- Select your desired date and appointment time and click 'schedule.'
- Your appointment has been scheduled! You can view your upcoming appointment under "visits / appointments and visits" and you will receive reminders for your upcoming appointment.
Using the MyCookChildren's App
- Login to your MyCookChildren's account via the app and select the child you wish to make an appointment for.
- Select "appointments" calendar icon, then select "schedule an appointment" green icon.
- Select "schedule with a provider you have seen before" or "tell us why you"re coming in." Follow the prompted questions and next steps.
- Complete the additional details section with a brief summary of the reason for the appointment.
- Select your desired date and appointment time and tap 'schedule."

MyCookChildren's video visits:
Cook Children’s pediatricians are partnering to provide online video visits to help provide the quality pediatric care you expect from Cook Children's. These visits can be done from the comfort of your own home on a smartphone, tablet or computer with a webcam. In preparation for a visit:
- Patients must have an active Cook Children's MyCookChildren's account.
- Patients must download the program Zoom to their device.
To schedule a video visit with your pediatrician, follow the below steps:
- Request a visit by selecting the "Appointments" tab in MyCookChildren's or by calling your pediatrician’s office to set up an appointment. These appointments will be available during regular office hours.
- When the time comes for a visit, you will go back to the “Appointments” tab in the MyCookChildren's app to begin your visit.
In addition to telemedicine visits, Cook Children’s pediatricians are actively working to help keep patients safe by direct messaging with patient families and providing phone visits when video is not an option. Messages can include a picture or video and can be sent through the MyCookChildren's “Messages” tab. Talk to your office and /or doctor to confirm if they are providing these options for their patients at this time.
Request medical records
Cook Children's keeps a record of each lab, x-ray, medical center, or specialty clinic visit. The privacy of this information is protected by federal and state regulations. This means your medical information is only available to the Cook Children's healthcare professionals caring for you or your child. Everyone else must get special permission (called authorization) before the information can be released.
Specialty care referrals
At Cook Children's, we provide more than 60 pediatric medical and specialty clinic offices to support your child if they require specialized care. We offer child-friendly medical and surgical technology, as well as care designed to help children and their families feel at ease and safe during a stressful time.
- Referrals to a specialist may be made by your doctor. However, no referrals are made until your child has been seen by his or her primary care physician.
- If you are a member of a managed care plan (HMO), a referral form is required for treatment by a specialist. Allow five to seven working days for the referral to be completed and mailed.
